Nomination Form
The nomination period is now open for this year's recognition program for the Physician of the Year Award , Nursing Leadership Award, Advance Practice Provider Award, Distinguished Leadership Award, and Lifetime Achievement Award.
A review committee comprising a diverse group of hospital leaders will evaluate all applications that meet the criteria. Awardees will be recognized at the Christian Hospital Foundation Gala at the Missouri Athletic Club on Saturday, March 2, 2024. Current Christian Hospital Foundation board members are not eligible. All finalists must be present at the Gala. Please read the award descriptions below and submit your nomination(s).
Nominations must be submitted by Wednesday, November 22, 2023.
Physician of the Year Award: Honors a physician who consistently upholds high standards in medicine, showcases compassion and commitment, employs innovative technologies or therapies, and demonstrates strong leadership in partnerships to improve community health in Christian Hospital's service area.
Paul F. Detrick Lifetime Achievement Award: Recognizes significant contributions and service to healthcare delivery at Christian Hospital. The recipient of this award will be announced at the Gala.
Nursing Leadership Award: Is presented to a transformative leader who encourages professional development and acts as a catalyst for positive change in the nursing culture at Christian Hospital.
Advanced Practice Provider Award: This is bestowed upon an Advanced Practice Provider who exhibits excellence through superior clinical skills, professionalism, innovative practice, interdisciplinary interaction, lifelong learning, and service to others.
